PART 2: ESTROGEN FACT 

After menopause, we don't just stop making estradiol because the ovaries slow down. We still have several ways to make it:

  • Estrone → estradiol

  • Estrone sulfate → estrone → estradiol — and estrone sulfate is actually a pretty significant estrogen reservoir.

  • Estradiol sulfate → estradiol

  • We can also make estrogen from androgens — testosterone can convert to estradiol, and androstenedione can convert to estrone and then estradiol.

So after menopause, estrogen production doesn't disappear, it shifts. Our fat, breast, bone, brain, skin and other tissues can still make and use estradiol locally from these precursors.

SO LET ME ASK YOU THIS:

    Q: What do these all have in common?

    Fat. Breast. Bone. Brain. Skin.

    A: They're all tissues.

    Q: And where can postmenopausal women still make and use estrogen?

    A: In the tissues.

    Q: Now where do we routinely measure hormones?

    A: In the blood.

And there's the problem: we're measuring what's circulating and assuming it tells us everything about what's happening in the tissues.

A blood lab of low estrogen is not etched in stone. It's meant to be measured against your other symptoms and especially Thyroid and vitamin D.

Going off blood labs is like looking at your bank's checking account and assuming it represents your entire net worth. You're seeing one compartment, not the whole system.

This is exactly why estrogen kickback can be so confusing to navigate: the symptoms are real, but a single blood draw rarely tells the whole story — especially post-menopause, when tissues are quietly doing their own estrogen production behind the scenes. Prolactin monitoring, symptom tracking, thyroid and vitamin D levels all matter alongside the blood work, not instead of it.
