Progesterone is one of the most misunderstood hormones in the body.
We often hear questions like:
How much progesterone should I be making?
How much progesterone should I take?
Is more always better?
And perhaps one of the most important questions:
Does the way progesterone is delivered matter (oral v. transdermal)?
The answer to all of these questions is more complicated than simply looking at a number on a lab report.
Your body does not normally produce progesterone as one large dose. It produces and releases hormones dynamically, changing throughout the menstrual cycle, pregnancy, and the years surrounding menopause.
That means when we're talking about progesterone, the amount, timing, delivery method, metabolism, and overall hormonal environment all matter.
At Platt Wellness, we believe understanding the *why* behind hormone balance is just as important as understanding the hormone itself.
Your Body Makes Progesterone Differently Throughout Life
In a healthy cycling woman, progesterone production rises dramatically after ovulation during the luteal phase of the menstrual cycle.
The corpus luteum—the temporary endocrine structure that forms after ovulation—becomes the primary source of progesterone during this part of the cycle.
Progesterone isn't released as one giant daily dose. Hormone production is continuous and changes throughout the cycle.
And then pregnancy changes the picture dramatically.
Progesterone During Pregnancy
Progesterone becomes increasingly important during pregnancy. Early in pregnancy, the corpus luteum is the primary source. As pregnancy progresses, production shifts increasingly to the placenta.
The placenta ultimately produces very large quantities of progesterone, supporting the uterine environment and helping maintain pregnancy.
Progesterone has several important roles during pregnancy, including helping prepare and maintain the uterine lining, supporting implantation and influencing uterine activity.

What About Menopause?
After menopause, ovarian progesterone production falls dramatically because ovulation no longer occurs regularly.
That doesn't necessarily mean that every woman needs to restore her progesterone to the levels she had at age 25.
This is an important distinction.
Hormone balance is not about chasing the highest possible number.
When estrogen and progesterone production naturally decline together, a lower progesterone level may be completely appropriate for that stage of life.
The goal should be understanding the individual—not automatically assuming that a low laboratory value means something is wrong.
The Bigger Question: How Much Progesterone Actually Reaches Your Tissues?
This is where things get interesting.
Two people could take the same amount of progesterone and experience very different effects.
Why?
Because **the amount you take is not necessarily the same as the amount your body can use.
Your digestive system, liver, enzymes, metabolism, formulation, route of administration, and individual physiology all influence what happens to progesterone after you take it.
And this brings us to one of the most important conversations in progesterone therapy: oral vs. transdermal delivery.
Oral Progesterone vs. Transdermal Progesterone
When progesterone is swallowed, it travels through the gastrointestinal tract and then undergoes significant metabolism before reaching systemic circulation.
This is commonly called **first-pass metabolism**.
Research on oral micronized progesterone shows that only a relatively small proportion of the administered hormone remains as circulating progesterone, while a substantial amount is converted into metabolites—including neuroactive metabolites such as allopregnanolone.
That isn't necessarily a bad thing.
In fact, those metabolites are part of why oral progesterone can make some people feel relaxed or sleepy.
Allopregnanolone interacts with GABA-A receptors in the brain and has well-established neuroactive effects. For some women, this can be beneficial for relaxation and sleep; for others, the same effect can feel like excessive sedation or fatigue.
This is why oral micronized progesterone is commonly taken at bedtime.
So Is Oral Progesterone "Bad"?
No.
This is an important distinction.
Oral micronized progesterone is a legitimate and clinically useful form of progesterone.
The issue isn't that oral progesterone doesn't work.
The issue is that oral and transdermal progesterone are processed differently by the body.
The route of administration changes the hormone's pharmacokinetics, metabolites, and potentially the experience a person has with it.
What Makes Transdermal Progesterone Different?
Transdermal progesterone is applied to the skin rather than swallowed.
Because it doesn't travel through the gastrointestinal tract first, it avoids the same gastrointestinal/hepatic first-pass pathway associated with oral administration.
But there's another important nuance:
You cannot always judge how much progesterone has reached tissues simply by looking at serum progesterone.
This is one of the most interesting findings in the research.
Studies of progesterone creams have demonstrated absorption through the skin, while serum progesterone measurements sometimes remain surprisingly low. Other measurements—including capillary blood and salivary progesterone—can show considerably higher exposure after topical administration.
One randomized crossover study of postmenopausal women found that after topical progesterone, serum concentrations rose gradually, while capillary blood and salivary concentrations were substantially higher than serum measurements. The researchers concluded that relying solely on serum progesterone could underestimate tissue exposure.
In other words: where you measure progesterone matters.
This is one reason progesterone therapy can be difficult to evaluate using a single blood test.
Why Timing Can Matter
Your body doesn't normally experience progesterone as one enormous bolus.
Natural hormone production changes throughout the day and across the menstrual cycle.
That's why some practitioners prefer dividing progesterone applications rather than taking one large amount all at once.
For transdermal progesterone, dividing applications may also provide a more consistent pattern of exposure.
One clinical study compared 40 mg of progesterone cream taken once daily with 20 mg applied twice daily. Both regimens produced increases in progesterone-related measurements, although the study was small and did not establish that one regimen was clinically superior.
So rather than thinking:
How much progesterone should I take?
it can be more useful to ask:
What amount, delivery method, and schedule make sense for my individual situation?
That is a much better conversation.
More Progesterone Isn't Always Better
This is an important point that often gets lost in hormone discussions.
Progesterone is a biologically active hormone. More is not automatically better.
Taking a large amount at once can produce a very different experience than using smaller amounts distributed throughout the day.
With oral progesterone in particular, a larger dose can increase exposure to progesterone metabolites, including allopregnanolone, which may contribute to sedation or fatigue in some people.
With transdermal progesterone, absorption can also vary considerably between individuals.
Research has demonstrated substantial variability in serum progesterone levels after topical application, and studies suggest that serum measurements don't necessarily tell the whole story about tissue exposure. Research clinical study here.
That's why individual response matters.
Does Progesterone Turn Into Cortisol?
This is another common question we hear.
Progesterone and cortisol are both steroid hormones and share parts of the steroid-production pathway. Progesterone can serve as a precursor in the production of other steroid hormones.
But the idea that taking progesterone automatically means your body will simply "turn it into cortisol" is an oversimplification.
Steroid hormone production is regulated by enzymes, tissues, signaling pathways, and physiological demand.
In other words, your body doesn't simply look at an extra molecule of progesterone and decide:
We have too much progesterone. Let's turn all of it into cortisol.
Stress physiology is considerably more complicated than that.
This is also why we believe it's important to look beyond the hormone itself and ask **why the body may be experiencing increased demand for stress hormones in the first place.**

Your Liver Still Matters
There's another piece of the hormone puzzle that deserves attention:
The liver plays an essential role in metabolizing steroid hormones, including estrogen and progesterone, as well as numerous medications and other compounds.
Hormone metabolism doesn't happen in isolation.
Nutrition, alcohol intake, medications, gastrointestinal health, metabolic health, and individual enzyme activity can all influence how hormones are processed.
That's why supporting overall metabolic and liver health is an important part of a bigger hormone-balance strategy.
But it's also important not to oversimplify this.
A "sluggish liver" isn't an explanation for every hormone symptom, and hormone therapy should not be used as a substitute for evaluating underlying health concerns.
The body is a system.
Hormones are part of that system—not the entire system.
Progesterone, Estrogen & Balance
Perhaps the most important takeaway is this:
Progesterone doesn't exist in a vacuum.
It works alongside estrogen and interacts with many other systems in the body.
This is why a progesterone number by itself doesn't always tell you how a woman is feeling.
Two women can have similar laboratory results and completely different experiences.
One may feel perfectly well.
Another may experience changes in sleep, mood, menstrual patterns, or other symptoms that warrant a deeper conversation with a qualified healthcare professional.
That's why we encourage women to look at the whole picture.
Age.
Menstrual status.
Ovulation.
Symptoms.
Estrogen exposure.
Progesterone exposure.
Stress.
Sleep.
Metabolism.
Medications.
And the route through which hormones are delivered.
What About Fertility and Pregnancy?
Progesterone deserves special attention when we're talking about fertility and pregnancy because it plays an essential role in preparing the uterine lining and supporting the early pregnancy environment.
Progesterone production changes substantially during pregnancy, eventually shifting to the placenta as the major source.
However, pregnancy is not the time to self-prescribe or independently adjust progesterone therapy.
If you are trying to conceive, are pregnant, have a history of pregnancy loss, or are experiencing symptoms that concern you during pregnancy, work with your obstetric or fertility care team.

The Bottom Line: Progesterone Is About More Than a Number
So, how much progesterone does your body need?
There isn't one universal answer.
Your body naturally produces different amounts at different stages of life—and the amount produced during a menstrual cycle is dramatically different from the amount produced during pregnancy.
But the most important lesson may be this:
Progesterone isn't simply about how much you take. It's about how your body receives, metabolizes, and responds to it.
Oral progesterone and transdermal progesterone follow different metabolic pathways.
Oral micronized progesterone undergoes substantial first-pass metabolism and produces neuroactive metabolites that can contribute to its sedating effects.
Transdermal progesterone bypasses gastrointestinal first-pass metabolism, but absorption is variable, and serum progesterone may not fully reflect tissue exposure.
And when it comes to dosing -- more isn't automatically better.
The goal isn't to chase the highest progesterone number possible.
The goal is to understand your individual physiology and find an approach that makes sense for your body, your stage of life, and your health goals.
